Family Support 

HPP offers a comprehensive approach to family stability through trauma-informed case management services that help families reach goals with one-on-one counseling.

Services include support with high-risk pregnancies, involvement with child welfare and/or substance use disorders, domestic violence, mental health needs, complex challenges to housing and/or a variety of factors that benefit from more intentional and long-term engagement.

Intentional Engagement

Families that receive ongoing case management services often face a variety of challenges related to high-risk pregnancies, child welfare, substance use disorders, complex housing issues and/or family violence. Family support is based on the Strengthening Families Framework, an evidence-based approach which seeks to engage, support and partner with parents in order to achieve the best growth and development in children.

Family Support Services

New Beginnings 

Individuals experiencing high-risk pregnancies receive intensive case management to support positive birth outcomes. Challenging factors to these pregnancies include recent substance use, homelessness, incarceration, mental health issues and other concerns. 

Domestic Violence Services 

Individualized case management and therapy groups for those overcoming and/or healing from interpersonal violence.

San Francisco Family Treatment Court

FTC is a collaboration of public agencies and community-based service providers that offers highly coordinated assessment, treatment planning, and recovery maintenance support for families involved in the juvenile dependency system as a result of parental substance use.

Keeping Families Together

Comprehensive support to families involved with or at-risk of involvement with the child welfare system.  KFT offers comprehensive support related to substance use disorders, housing and other challenges, and promotes stable, long-term family reunification.
