Full-Time Family Case Manager

The Homeless Prenatal Program is looking for a full-time employee who meets the qualifications of being a Supervisor and a Family Case Manager. This individual will spend time supervising employees in our Peer Mentor Program and providing clinical guidance to three case managers. The Peer Mentor Program is comprised of parents who were involved in the child welfare system before and are now providing peer support to families who are currently involved in the child welfare system. The Peer Mentor Program would require the supervisor to provide guidance and training in case management and group facilitation, boundaries, maintenance plans, and other pertinent issues surrounding families who are involved in the child welfare system. The supervisor will also be responsible for tracking and producing quarterly reports for the program.

This position also requires the supervisor to carry a case load of ten families at any given time. They will be responsible for providing guidance, extensive support and resources to families who are currently dealing with substance abuse, mental health and domestic violence issues. This position would be responsible for conducting intake interviews and drug and alcohol assessments. A reunification plan will need to be developed and each family connected to treatment programs, mental health services, and assistance with housing needs. This person must promote a positive, professional attitude towards families.
