Family Case Manager

The Homeless Prenatal Program is looking for a full-time Family Case Manager to work with individual and two-parent families who are currently receiving services from the Child Welfare System. The Case Manager is responsible for providing guidance, extensive support and resources to families who are currently dealing with substance abuse, mental health and domestic violence issues. The case manager will be responsible for conducting intake interviews and drug and alcohol assessments. The case manager will develop a reunification plan with the family and will be responsible for connecting the family to drug treatment programs, providing mental health referrals, and assisting with domestic violence and housing needs. This person must have strong writing skills and the ability to perform psycho-social assessments all while promoting a positive, professional attitude towards families at all times.
Background: Founded in 1989, the Homeless Prenatal Program (HPP) reaches out to families, women who are pregnant, and parents providing them with prenatal and postpartum education and support, counseling, referrals for substance abuse, domestic violence, and housing as well as support services. The HPP has a peer-delivered system of community outreach and education, which enlists the help of former clients, who, once trained, become effective community healthcare providers, as well as empowered, self-sufficient parents. Risk factors that are routinely addressed include pregnancy, low birth weight and prematurity, addiction, homelessness, poverty, and partner and street violence.
REQUIRED QUALIFICATIONS:
- Certified Addiction Counselor through (CAADE or CAADAC)/in progress
- Group Counseling Experience
- 3 years minimum of case management/supportive counseling experience
- A minimum of one year of experience in providing alcohol and/or drug education and information to persons with alcohol and/or other drug problems.
- Experience working with Child Welfare/Child Protective Services
- Must have experience working with homelessness/at risk families, substance abuse, mental health issues, and/or domestic violence.
- Ability to collaborate with team members & outside providers
- Must be dependable, organized, & detail oriented, also have experience with Reporting Requirements/Timelines
- Computer proficiency in Microsoft Office, excel, etc.
DESIRED QUALIFICATIONS:
- MSW / BSW
- Men and minorities are encouraged to apply
- Experience working with men, preferably fathers
- Capacity to ensure culturally appropriate treatment
- Experience working with families who have been involved with the Child Welfare System.
- Experience and knowledge working with diverse communities
- Able to communicate effectively both verbally and in writing
- If in recovery, five + years sobriety.
